Ohio State Fair Horse Show Schedule
Mandatory Horse Forms - club binder due May 15th
Club advisor must submit their club's horse form club binder to the Extension Office by May 15th
- Horse Project Information Form - one form per horse - charged $10 per week, per form late fee. Not accepted after June 1st.
- Mini Horse Project Information Form - one form per horse - charged $10 per week, per form late fee. Not accepted after June 1st.
- Participant Medical Release
- Permission to Participate Form
- Horse Lease Agreement - where applicable
- Horse Immunization Record - bring from your veterinarian's office, not a form we provide
- Request for Vaccination Extension - if necessary
- Shared Horse Form - where needed
EquiSTEP
This is the Equine Safety and Training and Educational Program that is required for all horse members, each year.
